When I worked at Costco it was 2 things

A guy with a forklift hit some pipes and broke them and i guess whatever he fucked up was enough that the store had to close for 2 days. Which at the one I worked at was unheard of. 2 days of a the store shut down at our busy warehouse was about $1 million in sales.

Tire center manager didn't lug a c5 corvette correctly and the wheel came off on the highway and totaled the car. Costco was found totally liable.

Neither employee was fired and both were demoted to cashier. That is how hard it is to get fired from Costco.

A company I worked for bought another company that sold a very expensive refrigerated pharmaceutical product. The purchased company shipped mini-fridges to all their customers, and stocked them with a year's worth of product. Year 1 revenue from the acquisition was pretty much zero.

Someone used a motorized lift to take a pallet of bbq sauce off the top steel out back and the whole pallet came crashing to the floor and bbq sauce everywhere.

Nothing that crazy but when I worked for a sales call center our account manager got fired for embezzling something like $30k. I don't think he got prosecuted though.

I was helping remodel a home. The owner shut off the water, the guy ripping up the floor removed the toilet to get the floor out, and forgot to turn the water valve off. Even with the water off, the residual water in the pipes leaked through the floor into the downstairs ceiling, which had just been repaired for a leak. The remodel was almost done, just had to lay the new floor, and that happened. The entire thing had to be gutted. (Super Ded fkn Srs)

I didn't see it but one of my servants accidentally lit a room full of money on fire

I didn't notice until my accountant's assistant pointed it out. At first I though I may have left a tip somewhere and forgotten about it

Transformer supplying 6600VAC three phase to an electric frac pump had the phases swapped, causing the VFD to trip on phase order. After a few hours of trying to find the issue, the owner of our company got impatient and intimidated the Hitachi VFD guys into swapping the incoming phases so it looked OK to the drive. Drive spun the 6000hp motor up to full speed, closed the breaker to put it across the line, and electrically slammed the motor into full reverse. Flames shot out both sides of the trailer, the floor was blown out, and turned a huge 3 phase contactor into a puddle of molten copper and fried a bunch of wires. Lucky it didn't hurt anything really expensive. No idea what it cost for the replacement parts and techs replacing the wiring and cleaning up all the black stuff.

A former co-worker who was in charge of wasting expired narcotics at the hospital I worked at costed the pharmacy 85K in fines when there were no receipts from the disposal company. He was keeping them and reselling.

I obliged a vendor master change request from a foreign vendor and the change I made causes our entire agency payment file to fail (roughly $50 million). Came back from lunch with multiple vms from senior leaders and the comptroller waiting at my desk lol.

Our agency cash position was inflated for a few days until the next payment file run lol

What a lot of people don’t realize is that its insurance companies picking up the tab for a lot of these losses. There is insurance for everything these days. Employee ran into a pipe/someone - Property or GL claim, CEO or major decision maker (or even employee) makes an error - E&O or D&O coverage. Someone at a company sexual harasses or even abuses a minor at there job - EPL or SAM coverage.

I was working in a restaurant in my early 20s. One of the chefs started a fire while cooking and the entire place had to close for 2 weeks for repairs. The owner said they lost so much money they almost couldn't reopen again.

The person who did it only kept their job for a few more weeks after it happened.

There was a DEI black guy at my last job that couldn't get any work done and the government client knew he couldn't do the work but he bid for a contract and they still gave it to him, a little over 5 million USD a year. He didn't pocket all of it himself, it was a 8 person team IIRC, but he hired his wife and his son so he probably pocketed at least half of it.

It finally became too much and he got fired after a year but he still won, made out with 2.5 million plus of the taxpayers money, lol.

We got contracted to check that the data scientists had designed their parameters correctly to identify and remediate the “affected class of persons” in line with a court ruling, and then sample test to confirm people were in fact being remediated.

Well I generally deal with delays and not directly with the costs so I don't normally see the numbers

One project I was on had cost over run of 200 million so I guess it's that.

It's routine for there to be delays I just normally don't see the dollar amounts

I work for a managed IT services company and one of our senior technicians accidentally deleted a local town governments entire production environment when deploying a new VMware disaster recovery environment.

Guys an extremely skilled tech, but we all have a colossal fuk up sooner or later.

My first summer job at 14, I walked out the wrong door and set off an alarm that caused the whole factory to have to evacuate for some reason. So that 30 minute paus in everything probably cost a lot. :(
