What frightens you most, misc? (srs)

Currently writing a supernatural horror. Trying to really create something terrifying, but coming up a lil short and looking for some inspiration from my brahs.

My best answer was "Sane, but trapped in a madhouse."

To tie into your answer you could have the main character believe he's sane, but he's actually a patient at the mad house. But he thinks he's a detective or something and all the staff goes along with his delusions. Would be great and never been done before.

I'm a big fan of the Lovecraftian cosmic horror type thing, but obviously that's very vague. Also, surreal stuff like Twin Peaks or Eraserhead.

A good Lovecraftian + surreal would be great.
