Hard to explain but for example if you go somewhere to eat 95% of the time nowadays the order is wrong. There seems to be a lack of critical thinking for basic tasks. Nothing runs smoothly anymore. Everything is made harder than it should be. What do you think is causing this?

Non-existent leadership. If an employee makes constant mistakes, the manager needs to work with the employee to fix the issue.
