Can someone please explain how Alec Baldwin is at fault for being handed a weapon with live ammunition on set?

Is this an "ignorance is not a defence" type deal?

If i leave an auto mechanic's shop and they clearly and intentionally cut my brakes and i crash into someone. Who is at fault?
