The entire system is setup with distributed responsibility so no one ever gets blamed for being negligent. However, when something this catastrophic goes wrong they are going to point at the head of the system. And given how many questions she didn't answer either she's completely negligent in her duties (and should be prosecuted appropriately) or was chosen because she would "look the other way" when those questionable calls were made by her superiors.

Regardless of her level of compitence she should resign from that position

She failed miserably at what is really her number 1 task. That position should be held by someone with proper field experience. She never should have been close to that position.
