What's the most expensive screw up at work you've ever seen in person?

We've got a job in Malaysia where the customer has requested to have one of our techs on board the rig full time. The operator will only let guys stay offshore for 28 days straight, so we have 2 techs rotating every 28 days. On about March 18, the customer realized that the tech that was out there was about to run out of days (but good on visa), but myself and the other tech's visas expire on March 27. So they have me fly all the way there to rotate the guy back to land on Monday to reset his offshore days, and then have him replace me on Friday. We charge them $3200/day/tech plus expenses.
I get there on Sunday, but my bag with all my PPE and clothes didn't make the transfer in Singapore and won't be there until Monday after the helicopter, so I get bumped to Wednesday. Wednesday we make the switch, and Friday morning the other guy shows up to the heliport smelling of alcohol so they bump him off the flight. I have to leave the rig anyway since my visa expires that day, but his bumping makes the helicopter late enough that I miss my airplane flight. Only seats available for rescheduling were business class.
So, $3200 * 9 days + round trip airfare (half business) from Houston to Miri + 3 nights in a hotel just to have me on the rig for basically 2 days.

One of our Windows Admins credentials got out, a hack group got in and locked a couple thousand servers worldwide with Ransomware.
Took us months to rebuild all that. They didnt pay the ransom, but the rebuild still costed a million dollars..
