More times than I wish.
Funny thing is, it happens always when I'm on a roll.
Not even the heaviest set in the day, more often while doing something like "why-not-one-more" descending/cool-down set.
As if somebody-up-there wants to warn me by telling me that nobody is invincible.
It never happens when you are in a slump and in a cautious mode.

However, the most important lesson here is, "Never Quit."

Irreparable shoulder damage. Have been to countless physios, had MRI done, on the surface there's no major tears or anything but my shoulders/upper body as a whole is so warped from fcked up training that my shoulder seize up and can leave me in pain for hours if I even attempt a pushup, pull up, stiffen if I do OHP, and I think the last time I did pain-free back squat was back in 2011.
Have legit cried over how bad it is. Imagine if virtually anything you did for upper body caused your tendons to inflame and your biceps/shoulders to seize up. Even rehab stuff, ironically. Rowing machine as cardio and for minor back stimulation? Nah, causes pain. Face pulls to strengthen rear delt and rotator cuff? Nah, causes pain. DB rows to strengthen the back and help things? Causes pain.
The irony is that, in chasing a strong body as a teenager, I'm now frail, weak and even physiotherapy exercises cause pain, let alone actual exercise.
