Cost in India would be ~10-20% of the US per participant. And not as many participants would be in an Indian trial. It would have cost Pfizer $100-200 million dollars to have access to the Indian market, a country 4x US population. India refused to let Pfizer be immune from prosecution for adverse events, so they withdrew their requests after trying for two years to be immune from prosecution.
He clearly understands this, he’s just trolling as usual. Cost of trials is of no concern, especially to a company taking in record profits at the time. Their concern was 100% about being held libel for their damages and not being able to pull their usual fukkery with regards to skewing results to exaggerate benefits and downplay(or even lie about) risks. It was 0% about concern of clinical trial cost.
