I think it started when some pizza locations pushed the 'premium' stuff on the pizzas, and then major chains seem to have followed. I mean, since when was a medium personal pan Supreme pizza from Pizza Hut almost 30 bucks (EU prices). Did a double take last time I wanted to go there - then walked on down to the steakhouse nearby where I had USDA striploin for almost the same price...
