While physical attraction is very important, if you've agreed to marry someone the idea is you plan to spend your life with them. It's inevitable that you will both get older and less objectively good looking, but hopefully still be attracted to each other because that's just the way it is.

Getting fat is different, it's something you can control and if you choose to stop having control over it you are showing you don't give a chit about your partner or their attraction to you.

I suppose. But suggesting drugs first before surgery and without an ultimatum if and when it happens seems more reasonable to me.
