Doesn't necessarily have to be a gory movie. Just one which unsettled to you to the point where it was either hard to watch or just made you feel strongly disturbed and had a lasting impact.

Movies like A Serbian Film are so grotesque that they become black comedies IMO whether intentional or not. Same as the likes of Human Centipede 2, the entire premise is so over the top that you can't help but laugh at the ridiculousness of it all. I mean how many times can you say, 'Yea remember that scene where the woman gives birth in the car, then slams the gas pedal crushing its head to escape a psychopath who jacks off with sand paper'.

The one truly disturbing movie I can recommend isMartyrs.NOT the US remake. I found that unsettling because the premise was obscene, but it was delivered entirely realistically and not in that over the top funny way. The plot was quite intriguing and had some stunning visuals, albeit sadistic in nature.

Edit: Forgot to includeWe Need To Talk About Kevin. Not gory at all but its a movie about a boy who grows up to be a school shooter, and how he was basically evil and messed up from birth.

can't even imagine how sick the producer must have been to even make a movie like that.

I'd tell you but I forgot the name, it was on Netflix, some other language film. It was about a guy being obsessed with some girl, kidnaps her, does pretty dehumanizing things with her (one of which was some weird sort of gag machine that looked horrific). In the end she starts torturing him instead lol.

I'm not impressed by "a serbian film" or "murder set pieces" or any of these random sicko movies depicting ridiculously gruesome stuff. For me to care, the movie has to be good, whilst containing elements of the disturbing.

By that token, Donnie Darko is the obvious GOAT. Pan's Labyrinth is also a great contender.
