MRI RESULTS of entire spine for 4+ years of extreme widespread chronic pain (18 ). :/

18 means I'm 18 years old.

Sooo, here's the update. I guess it explains the extreme headaches.

MRI - Cervical - Excessive wear/degeneration of discs, Narrowing of the nerves going out of side. Pinched. Sciatica. All discs affected.
. Thoracic - Wear and tear
. Lumbar - Excessive wear/degeneration of discs. L1-S1 affected sciatica.

X-Ray - Lower back.
- Couldn't see much, requested MRI.

Shoulder Ultrasound - Unremarkable appearance of the joint.

Further: Podiatry, Pain clinic for inections, ask spinal team for surgery, physio, maybe etc.



:/

What can I do? I'm not "depressed", not at all, just going insane trying to figure out what's going on. I pawn it off as posture issues. Just extreme ones.

I’m sorry you’re dealing with all of this at 18. You have the MRI read of someone in their 50’s. You may have congenital issues with alignment, as well as poor posture. Are you a manual laborer, and if so have you done this for several years? Poor gym routine? What are you doing to cause extensive degenerative change? It’s tough for me to weigh in too much without being personally involved with your case.

You mention sciatica with your cervical read. Sciatica is inflammation and pain due to compression of your sciatic nerve leading to burning and shooting pains in the lower extremities. This generally takes place in the lumbar region, but you me mentioned it in your cervical section of MRI. Was this a typo? Although it’s possible for cervical impingement issues to cause sciatica, it’s relatively uncommon.

Have you talked to your physician about this? What kind of doctor are you dealing with and what’s their current recommendation? What medications are you on at the moment? Have you begun physical therapy?

Unless your quality of life is 1/10 or there’s a risk of acute cervical instability a neurosurgeon will not touch you. Evaluate, sure.. but they won’t bring a scalpel anywhere near you. Pain management may be helpful by providing epidural corticosteroid injections. Do not get started on narcotic pain control at your age. It would surprise me if any physician worth their salt offered you such an option.

MRI's don't always explain pain. Pain can be very complex. a guy with a clean looking MRI could still experience pain. while a guy with the MRI of an 80 year old cripple could just be going through life without much issue.

Not saying what your MRI says doesn't explain your issues but it often doesn't provide the entire picture.

surgery should always be a last resort. you probably need to work with a physio/rehab specialist that really knows about spinal/neck related injuries/pain over some months or whatever.

This would normally be where I would recommend the standard joint supplements (Fish oil, Glucosamine with Chondroitin , etc) but that is for minor to moderate joint issues. Still would look into taking those, but you're right man you have to get to the cause of this issue.

Any chance this is a viral or bacterial infection? It would be rare I think, but I am just throwing ideas out there.

I'll answer everything in order as you wrote it.

1. Not a manual laborer, don't really lift stuff as it worsens the pains.
2. Perhaps a poor gym routine, maybe its imbalanced, started "lifting" at 15ish by doing weighted pull ups and dips as a standalone routine. Added in bench, but that was all. I later did squats and deadlifts (2ish years later)
3. I have no idea what I'm doing in terms of causing the degeneration. I mean, I do have pretty severe APT to a point where my feet pronate. My neck posture wise has been a problem area since day 1.
4. Hmmm, the rheumatologist clearly said sciatica in the neck, maybe that was a mistake if it doesn't make clinical sense. Perhaps she was talking about the pinched nerve? I will ask again.
5. Physician? Who is that?
6. Sooo, over the years I've seen over 10+ specialists/doctors. The main root cause finders being the neurologist and rheumatologist. The gp tells me to wait in line really, widespread chronic pain is a mystery.
7. I have tried over 15 medications, all useless with stupid side effects. If they worked, I'd take it 24/7. (I'm not prone to do drugs I don't even like breathing in the air of smokers in public lol)
8. I tried physio a while ago, but I want a second shot at it since I think it's all postural, and it's just gone way out of hand.

My quality of life is 0/10, but I know what u mean. I'm half dead all the time and everyday my body is on fire. It's insane.

Muscles are pretty powerful. If a surgeon tries to operate, it'll just whack out of place again. I'm not too sure though.

Thanks for the advice. Why not try narcotics or some pain drugs? I know there's a risk of getting "hooked", but I don't care at all.

I had the same chit as you

Even younger. I had to take freshman and sophomore in high school year off of baseball because of it. You prob don't understand how much baseball I played. At one point I was on 3 teams. Aau, little league and jv in 7th grade

They also told me I had an old man's mri when I was young.

Went to a neurosurgeon. They said I was so young they didn't advocate surgery

Anyways, long story short is I did physical therapy which was straight ab work.

When I was like 19 I hurt it squatting. Thought I fuken broke my back. Felt it tingling in the center. Went to the er. Argued with the doc, she said it didn't sound like nerve pain. I was like "I ALREADY KNOW I HAVE NERVE ISSUES. SO I KNOW ITS ALREADY THERE". I only got an xray, no mri. She told me I could go back to work in 2 days and the gym in 7.

I did. Think it got better. But later on I hurt it at the gym again. So I stopped the gym for a bit.

I then moved and hurt it again working out. So I got an mri

And my 4 bulging discs had turned to 8. With more chit. I had muscle strains on top of muscle spasms

Idk what happened but it seemingly doesn't affect me in real life. Every now and then I feel it.

Last time I went to the doctor for it they told me that I just have to stop what I'm doing anytime it hurts

But it doesn't prevent me from doing anything day to day. It also crept up my neck and affected my shoulder too. I had to stop playing baseball for fun cause I was at a game where I couldn't pick up my glove and swinging feels like my shoulders pop out of their sockets

I am scared to see what I'm at now tbh

But I'm prob wayyyy more active than your avg Joe. Im a tradie, work out, etc. And it doesn't affect me. Only some shoulder stuff - I can workout shoulders and dumbbell press unaffected but barbell press hurts my shoulders. That's about all I can think of that hurts. Even standing for hours and hours I can vaguely feel my back

Prob equivalent to pains that pop up anybody feels

My GF has had Lumbar degenerative disc disease since like 2007. She has an MRI proving this in 2008.

Doctors in Philly didn't want to treat here and said her MRI records were destroyed and they didn't believe her and didn't want to give pain meds.

I have worked in healthcare for 33 years and thought this was total BS. So, I just got her records from the company that keeps them the other day! They prove she has the Lumbar issue.


She has been in increasing pain for years, her right leg now does not work right so she can't lift her foot, she can't work and can barely think straight due to pain. So, I was able to think for her.

Now I have her hooked up with a pain clinic, later this week. After that, we need to see a surgeon.

OP, what are your plans?

My gf is incapacitated in so far as normal activities go and if her back can't be fixed her life will be ruined. It's affecting her psychologically, in a VERY bad way, and physically she can't even do fun things let alone work.

What is your plan?

Yeah, I think that too. The results from the MRI showcase the result of something else. Ie, a log falling on a leg causing a fracture. They told me that at the beginning, that people with perfectly straight cervical spines can encounter 0 issues in their lifetime.

Why do people say to avoid surgery? Sounds like a quick fix/patch up.

Yeah, I'm seeing to the physio stuff, just don't know what else they can offer, by my understanding I should've completed everything physio has to offer already when self posture fixing lol. Eh, I guess they're the specialist.

Opioids at this point in your life should be one of the last options. You may get significant relief from Neurontin or Lyrica. You can talk to your rheumatologist about this (unless you’ve already tried them for at least 2-months and had little to no symptomatic relief).

What did the blood work your rheumatologist pulled say? Did he test you across the spectrum for autoimmune issues that may be causing your chronic pain? Although this sounds primarily musculoskeletal in nature, there may be an autoimmune component. Do you have autoimmune issues in your family (this may include type 1 diabetes mellitus, as well as thyroid disease)? Also, have you ever been in a car accident/neck accident of any sort? Straightening of the cervical spine often indicates prior trauma.

If you have MSK imbalances you need to visit a quality physiotherapist (physical therapist). They sure as s_hit are not all the same. They will help you build a proper foundation. It’s crucial to listen to them and be extremely proactive at home with whatever plan they provide. This will take work and discipline. Everyday. Possibly indefinitely, but you will improve.

If you have a component involved such as fibromyalgia it’s important for you to train yourself in meditation and stress relief. As well as possibly a medication called Cymbalta. These are things that need to be discussed with your family physician and/or psychiatrist.

You may benefit from a referral to a sports medicine physician. They are trained best to resolve MSK issues and would likely point you in the right direction for a quality physiotherapist. They are also trained to provide trigger point injections to those “knots” you spoke of. This will help release the muscle spasm in your traps which hinder proper posture, as well as catalyze the healing process your physical therapist is working to accomplish.

Regarding surgery, this is a last resort and surgeons don’t want to cut someone like you because more likely than not you will get worse and not better. Think of your spine as a building scaffold. If you patch one part, another area will need to be re-enforced differently. You do not have pathology indicating the need for surgical intervention at present time.

Your situation sucks, but you will need different specialties to obtain a healthy baseline. It seems like a lot now, but reread what I typed up a few times, maybe take notes and educate yourself for each appointment. Work as an advocate for your health with the different specialists you visit.

Bro listen to me, don’t give any of those tests credit many people have those issues and no pain. What you need to do is immediately order the book healing back pain by John Sarno. Then research him and his concepts, I was in your boat.

Check the reviews on Amazon first if you don’t believe me.

Sounds to me like your gf's disc ruptured. Even though my chit was bullchit I was lucky it was only bulging discs

And the discs didn't explode. If your gf is in constant pain, it's possible a disc ruptured

With a bulging disc it's only when the disc bulges and compresses the nerve. But if it ruptured the pressure on the nerve can be constant

BUT I'm just offering suggestions. Not a doctor

Hmmm, doesn't quite sound similar. Mine wasn't brought on by injury, or anything at all honestly.

I was extremely active too, did tons of sports. Sucks how our bodies imploded from healthy exercise lol.

Did that ab work in PT even work?

Wow, 8 bulging discs?? At that point your lumbar is just curved different lol. That was brought on from a max effort squat?

Check out kneesovertoesguy hear what he has to say about healing things through movement and strengthening muscles that are never used and such. Could help, seems like doctors aren't doing much for you at the moment.

With a bulging disc it's only when the disc bulges and compresses the nerve. But if it ruptured the pressure on the nerve can be constant

I am not sure.

I know that one of the lumbar discs presses on her spine and that parlyzes her right anterior tibialis and that muscle, over your shin picks your foot up. So, she can't do that, thus she has trouble walking. When she does all the muscles in that leg get stressed and are painful.

Are you getting any kind of surgery?

What is your plan?
In this order as to not get pushback from insurance and to expedite the diagnosis/treatment of your girlfriend’s issue:

1. Make appointment with her family physician. Not an urgent care center, not a mid-level.

2. 2008 was a long time ago. She needs orders for new lumbar XR and new lumbar MRI. She may benefit from trial of physical therapy, as well as prescription NSAID & muscle relaxant (I don’t know her medical history, so what she can tolerate will be discussed with her family doc, etc).

3. Depending on MRI result, she will need referral to pain specialist and/or neurosurg. If indicated, go with epidural corticosteroid injection before narcotics. Neurosurg would agree with above unless they observe something on MRI indicating urgent surgical intervention.


FYI, any issues with numbness in her taint, or bladder/bowel incontinence means she needs to go to ER for emergent evaluation and MRI.

Wow, that's a really long time :/

How did her issues start? Was she lifting at all? (I'm not diagnosing here, I'm drawing parallels to myself, my pelvic/lumbar region issues may have been beought on by posture/lifting or simply a manifestation of issues travelling down the chain starting at the neck.)

A surgeon? Have you already consulted with them? What did they say they can do in terms of fixing her?


My plans? Sit on a waiting list for all my referrals. Have another try with physio, follow it insanely closely. Start doing RDLs and doing weighted planks, (Ie, protecting the lumbar) never bothered in the past. I may be missing something, but that's the jist of it. Honestly, someone in an old thread of mine mentioned opioids/opiates? Not sure, but it is tempting.

What about your partner, other than surgery, what has she got lined up? Actually, how old is she?

Yea, mild canal stenosis (narrowing of spine) and other chit like I mentioned

It felt like I pissed down the back of my leg. I was like 13 or 14

I bent down to pick up a baseball at my bros game.

They think it MIGHT have been my heavy backpack in middle school

It's hard to describe. Yea I do think ab work helps, it completely supports the spine. So improving them won't hurt

Idk how to describe how it affected me because seemingly I'm able to do everything. I'm cognizant of it existing but it doesn't hold me back most of the time. Every now and then I will feel it but then so will most people standing up on concrete for 9 or 10 hours

It's NEVER gotten to a point like it was when I was a teen. I couldn't walk at certain points. My legs would go numb

I've only ever felt slight tingles comparatively.

Has it gotten better? Id prob have to get an mri right now to find out. Never experienced what I used to but I know it still exists. So idk about "better"

I'd say the overall experience has been manageable with times of discomfort
