Getting stabbed in the back by a person I trust. The phobia would be not wanting to get to close to anyone for fear of it happening again.

I've got a remote controlled rat. I set it up to run out from under the table at work in front of a chick, not knowing that she had a huge phobia due to a bad experience with a rat when she was a kid. She was a basket case for about 15 minutes. I really regretted doing it and never would have if I knew about her phobia, but it still was pretty funny.

That blue abyss when you dive under is the stuff of nightmares, just thinking that sharks and God knows what else can be lurking just out of sight and you wouldn't even know is enough to make me piss myself.
I used to live on an island in the South Pacific (Tuvalu) and the reef on the ocean side drops off almost vertically into the abyss. We're talking 6000 foot depths within sight of land. One day I decided to snorkel over the edge of the reef at high tide and it creeped me the fuk out. The water is so clear it's almost like looking through air, and the reef just drops straight down until it disappears into the darkness a couple of hundred feet down.
