While that does mean the pilot/s in a lot of situations, it could also be the maintenance or air traffic control.
In a medical unit I was flying for the company commander and one of the warrant officers crashed as a result of misdiagnosing an engine malfunction. They had an engine failure, cut power on the fully operational engine instead of the broken one, and down they went. The commander died. The warrant officer was the one who performed the emergency procedures. That has to suck to live with.
Anytime there is a crash, the US Army Aviation Center of Excellence will do their investigation and identify all contributing and non contributing factors. Then they go around to each unit and give an in depth brief on what happened and why. So we could learn from it. We would listen to the black box information, see the aircraft configuration and watch it play out. Listening to the voices of dead aircrew members was having a pretty big impact on the communiity so they started using computer voices instead of the raw data.
When I was at Rucker, this Louisiana Guard unit working MARSOC crashed two helicopters in the gulf. It was nightmare. Listening to their voices recordings and understanding the situation after the fact. Those pilots killed all of those people unnecessarily because of ego. I mean, a fucking nightmare.
It is one of those things, no one has any delusions about the danger. Everything an aircrew does is literally designed for the investigation after the crash.
Mission planning documentation, filing the flight plan with ATC, mission briefing and approval process. All of those things are done knowing the documentation is there specifically in case of a crash.
I have experienced my own mechanical failures and I have observed people behave like complete morons in the cockpit. You would be surprised man. People get complacent
