Bureaucratic negligence at the hands of state and federal government(s) as well. There are too many agencies/departments both state and federally to ever properly keep track of potentially dangerous individuals especially if they're moving around. The current system will NEVER stop these shootings from happening. Each state has state and municipal police forces, health institutions, and military branches in addition to federal offices for institutions like the FBI, NSA, NRA, etc etc. Multiply that by 50 states and expect them all to share red flags about potentially dangerous individuals. To ALL the firearms dealers. There's a reason why the background checks are a joke in the US and why the shooters are almost always known to police or health authorities somewhere after they've already blasted off. There has to be a single centralized reporting centre when it comes to firearms in the country or these people will just continue to walk through the grand canyon sized cracks in public safety.

It goes both ways btw, not all guns used in crime are reported either. We only know what we know, and current statistics show guns are used about 10x more for crime than they are for self-defense.

Almost every rampage killer in the West, whether they used a gun or not, whether they were a Christian, Muslim or atheist, has had a history of drug use. Usually legally prescribed SSRI anti-depressants but also steroids and cannabis. But according to conventional wisdom mind and mood altering drugs can't possibly be linked to senseless acts of ultra-violence. The link must be the weapon used. Or poverty. Or some such thing.
