I write my workouts down in a notebook, but I don't take the notebook with me, nor do I record the weights I lift. I memorize my entire workout so I know how many reps & sets I have to do. When it comes to the weight, I do what I can do. I find that shooting for a weight is less useful than using a lighter weight but getting a quality rep. The weight goes up when it goes up, but my form is way more important.

This. My current notebook is almost full, dates back to sometime in 2017 I believe. Every set for compound movements gets logged, weight and reps. Accessories I track a little more loosely but still put enough info to know what I need to hit to progress.
