They need to be shamed and ostracized.
The moment people start treating them like the disgusting ogres they are is when it will change.
Also instill some form of cultural fasting, like ramadan, this would help tremendously.

On the responsibility part - I don't think it is "kinda". The abdication of responsibility pervades many aspects of modern society - ironically, the very society most of these people blame for their ills.
Anyone on this forum who has had to "cut" or adhere to a reasonably strict diet/lifestyle for gains may have very little sympathy for the obese. Especially the posters here who were once chubby etc.

There’s obviously not one right answer, but proper physical education in schools (starting from kindergarten), including ethics/mindset education, physical training, nutrition, and compulsory participation in competition wouldn’t hurt, with an hours per day (not per week) target. It should be the most important part of the school day, we all know the character/mindset built through hardship is what allows you to achieve in the future.
No idea about the current generation, but I imagine when parents are put to shame by their 8 year old sons/daughters, there might be a culture shift.

Culture. It all starts from within the home. When you implement a healthy lifestyle in your home, your kids follow suit.
There's a reason why America is so fat in general, but sub groups like asians and the amish aren't.
I watched a documentary called "The Motivation Factor" on YouTube (yes I actually rented it haha), and it talked about the importance of movement. The premise was based around the gym classes in La Sierra High School. Of course we all know it was partly to get kids in shape for the military/war if necessary, but the beneficial effects it had were compounding in their lives.
We don't move enough, coupled with high caloric content. So many people grew up with zero discipline in health and fitness, so this just creates the perfect storm when you add in the capitalistic pursuits of fast food.
Start young. Create an in home healthy living lifestyle. Schools need to promote major health education/gym restructuring to push kids physically. This is just as important as mental stimulation, if not more.
Until then, people will continue to get more unhealthy, and continue to implode the healthcare industry.

Top 1% should pay their fair share. They got all that money to eat they won't miss a little.
