My younger brother is im the ICU after eso****eal varices and alcohol withdrawals
And here I am on the other side of the country at work not skipping a beat because that’s what’s expected of me.
Mind is racing dunno how to respond to this.
None of it makes any sense to me.
No more relaxing drinks with my bro when i go visit even if he makes it through.
EDIT: I actually don’t know how to deal with something like this. Do i tell people or keep it all bottled up until i spaz out.

My maternal uncle died from his alcoholism @ age 40 srs.
He had about 10 episodes like the one you've described before he finally succumbed to his poor choices and weakness.
I don't say that to be crass, I say that because it likely means your brother will survive and you and your family will have a chance to intervene in his lifestyle before his mistakes become life-ending.

alcohol withdrawals are no joke. Saw it first hand with my inlaws.
IIRC Alcohol is one of the few drug withdrawals that can actually kill you.

A lush/drunkard uses alcohol as a means to an end. For an alcoholic, the booze is the end in and of itself.
My uncle stopped going to his job that my grandfather got him so that he could stay home and drink. He was eventually reduced to living in a public housing facility in Inglewood and scraping together enough spare change to hit up the licka sto and buy a cheap bottle of some paint thinner tier liquor-- I forgot what his brands were already, he died almost a decade ago.
He would show up to family dinners so drunk he would have pissed himself before he came in and not even realized it/forgotten that it happened. He would drink so much that he would pass out on the toilet when he was visiting. On the rare occassion he was sober, his hand would shake so much that he would stumble and fall as the cane he has to use to walk (for no reason other than CNS damage) would slide from under him and he would collapse.
Be an adult and stop drinking for 2-3 days. Most likely you will experience some minor headaches but no major physical consequences. That is how you know that your "addiction" is a mental issue and not a physical one (as alcoholism is). Now you have the tools to solve it, so solve it.
BTW I say all this as a major fan of the drink.

He isn’t a text book addict either. He has quit cold turkey with no problem but goes back in times of stress.
The worst part is I know him. I lf he had any worry signs he would have quit asap

Damn hope your brother pulls through. Hopefully this is his rock bottom and will get clean and sober. He needs to go see a therapist for his issues. He's just trying to drink his demons away and thats not how it works.

Most of what you would be doing without gaining some knowledge first is enabling him.
My mom is an alcoholic(in recovery) and did some dumb **** multiple times, I never dropped a thing to go visit her. That was her choice to put herself there and I wasnt going to let her get the ability of having me feel sorry for her for a second or effecting my life. It just enables them in so many ways to keep doing what they do. They need to learn to become RESPONSIBLE for their own actions and emotions. It isnt your job. Their behavior negatively impacting your life is like a hard on for addicts. Gives them so much more reason to keep doing what they do.
Notice how I said she is now in recovery and has been for over half a decade now.
