Not really sure what the "gotcha" is suppose to be. Its just human error. You can go to a restaurant and get food poisoning. You can buy a bramd new car that has a recall on its fuel pump. Hell, you can buy a seemingly harmless drug like Ibuprofen and get deathly sick from a tainted excipient layer. I don't know anyone in the medical community saying the vaccine is "flawless" nor is this story proof that the vaccine is otherwise dangerous. If you believe that, you've fallen for the medias fear mongering.
FYI even with FDA approval no drug company guarantees 0 adverse reactions from a drug or vaccine. I think that even if Pfizer vaccine gets FDA approved this fall, you'd move the goal posts to some other excuse for not getting it.
