Dude has his facial skin flayed off, his hands chopped off, and they stick a machete through his mouth to hold him still and they cut at his throat with a dull knife ... He instinctively tries to touch his face as well, but he cant because they've chopped off his hands ... It's pretty damn dire, and he's alive the whole time because they've injected him with morphine/drugs which you can see as a couple syringes are injected in his chest.
