Are you sure you're talking about the bone structure or just the face? When people gain weight, the skin stretches and looks fuller. When they lose weight, the skin looses up and they end up looking a lot older, albeit much slimmer.

Loose skin. Even if they get it removed it takes a very long time for it to look normal afterward.
