Antifungal creams don’t work like antibiotics- you have to use them for 4-6 weeks to see the effects. It has to do with their mechanism of action, but just trust it. Use it twice a day for 6 weeks and it’ll go away, or you can neg me into oblivion

As everyone else said keep using the anti-fungal creams. Also buy and take oregano oil (pill form is fine). Keep your feet dry. You can buy sprays as well. Change your socks frequently. You can even buy socks that help mitigate chances of getting fungus by staying dry much better.
Also take a day you aren't doing much and disinfect all your old shoes and dry them out extremely well.

On the weekend it usually dries up

What kind of work do you do? Do or have chemicals got on your shoe that could be causing allergy? Have you used the same boots for all this time? Have you tried changing boots? It does not look fungal to me. I also recommend new boots.
It looks more like an allergic reaction (dermatitis) to me. It should be very easy for a dermatologist to do a KOH test or send in a scraping.
If you use a steroid for more than 2 weeks it can cause an ulcer.

It does look fungal. Been there before. Does the red marks turn more brown and then the skin peels off after?
Wash your boots down with a bleach solution and stick a garden hose in them and flush them out. The fungus lives in the lining and insulation in your boots. You can change your socks several times a day and it won't help.
Go barefoot as much as possible after work.
This chit will kill the fungus and cut down on the itch. You can get it at most tack shops or farm supply stores. Stick to the original formula. Not the gel.

You can get the absorbine liniment for people in the drug store abut it's not as good and dries out faster.

Did not see pic but will use my imagination. Switch to merino wool socks (I wear darn tough) and leather boots. If your wearing synthetic, goretex, or waterproof lined boots your feet can’t breath properly.
