Happy for you, OP. Seriously. I would do anything in the world to be a father. I thought I was a couple months ago, but alas, it wasn't meant to be. My whole life would be spent loving and providing for my partner and child. I pray you both don't choose to abort. If you don't, she must quell her drinking, and you should as well to support her.

Tear/defect maybe...we were using them all the time.

It happens...

My gf got pregnant and had an IUD. Expulsion (when IUD partially or completely falls out of the uterus.) So rare for that to happen but it did.

Ive always had complete faith in condoms

Out of all the guys i knew growing up who knocked a girl up all admitted to blowing inside

It was never this " i used a condom , dunno what happened "
