If there is, then there is an infinite number of possible forms “life after death” could take. We could all be reincarnated, or become one with the universe, or lose all our memories and relive our lives eternally, or any incalculable number of things.

That’s true, it’s hard the “hard problem” and all we have are guesses. However, our lack of knowledge does not therefore necessitate a supernatural explanation, nor does it follow that life exists after death simply because we don’t know how consciousness works.
