Saved A Guy’s Life Tonight (srs)

I know it’s my job and all, but because of half the chit we get sent to, it doesn’t happen frequently enough. This particular one I wanted to share.

I went to a 59 year old man who apparently looked like he was having a stroke and was called in by his wife.

When I got there, he was unresponsive and not breathing; in cardiac arrest.

What was unique for me about this job, was that I shocked this guy a total of 9 times, which is the most I’ve ever shock someone. We managed to get ROSC (return of spontaneous circulation) and him breathing for himself.

Once we had ROSC, his ECG was showing a clear, inferior STEMI (heart attack).

We got him into hospital and stabilised.

I checked back, a few hours later, he had been to the cath lab to have the blockage removed from his heart, before being admitted to ITU.



You can see how all of his observations started to look good after the 9th shock.

How long did it take from initial 999 call to first shock? Nice work though, made his family a whole hell of a lot happier than the alternative I’m sure.

So luckily we got stood down from some other bullchit job and was about to go on meal break, but got auto allocated this as soon as they categorised it as a C1 (life threatening) job, about 3 mins from this patient. Got there, recognised it was an arrest within 30 seconds, applied pads,, checked rhythm, shocked. So maybe 5-6 mins from time of arrest (it was witnessed).

gj. what's the most bs call you've gotten? here in the USA I see people comming to the er via Ems for preg tests, thats probably the worst I've seen.

How do you figure that you saved him? It sounds like you were just holding the handles of the little machine. I've put more effort into holding a door open for an old lady, but you don't see me on here bragging about how I saved her life.

Seriously, though, she was slow af and I had to stand there for a long time.

Got called to a guy who’d burnt his lips on his coffee (came through as airway burns/compromise).

The pads are actually adhesive now so I don’t even have to hold them. But seriously, in between shocks is 2 minutes of mechanical CPR, creating a patent airway, cannulating, given age adrenaline/amiodarone/fluids, full primary survey, suctioning the airway when compromised, creating an evacuation plan, so a little more than simply pressing the button.

Yes pretty much for about 70% of the overall arrest. We managed to get a LUCAS on scene eventually via helimed, which is a machine that does the compressions for you.

We had quite a lot of different rhythms throughout, it was fascinating to see.

Started off with vfib for the first 3 shocks, then briefly asystole before going back to vfib for another 5 shocks. Then ROSC for 15 mins before losing a pulse but having PEA. 2 more cycles of CPR/rhythm checking, then pulseless VT before our final shock that got us ROSC again which maintained all the way to A&E.
