How long does it take to start doing 5+ reps of your max usually?

I made good progress just doing linear training working in all rep ranges, basic exercises, trying to eat and sleep well.

Depends how advanced you are. It doesn't take long if you are beginner. Maybe a year as an intermediate and if you are advanced it can take a very long time. I'm not sure where I'm at in the progression but I'm still putting 5 pounds onto my lifts every few months when training is going well. 3 years ago my deadlift max was 660. I can rep that now on a good day. I repped 675 a month or 2 ago for 2 smooth reps.

Also this is assuming you don't train like a pussy.

Imo intensity > volume. So i like to train sets of 6-8 as I'm working up to my top set during my warm up. Then at my top set I'll train to failure. Sometimes I'll do 5x3 on my heavy compounds and train my last set to failure if I can do more reps than 3. On isolations I'll do like 3x8 to get a pump and train my last set to failure.

Not sure what your goals are but I like to train the big 3 (squat bench deadlift) and also build muscle. So a bench day would look something like warm up to a smooth heavy single at like 90-95%. Then back off and train some heavy triples to near failure. Then do another backoff set with like 70% (something I can do for like 10-12 reps) of 1rm to failure.

So bench, then I'll do incline db bench 3x8 last set to failure. Then db skull crushers 8-12 last set to failure. Heavy tricep pushdowns jay Cutler style 3x8-12 last set to failure. Then dumbbell lateral raises 3x8-15 last set beyond failure using half reps.

Idk I don't really use a program. I mostly go by how I feel each day as to how much weight I use and sometimes back off on the amount of volume if I'm shot. But use the train to failure concept on almost all my lifts. My split looks like bench, deadlift, squat, rest, upper body push/pull, rest, rest.

I do doubles and triples often. Often at 2-3 rep max. Do you recommend training those away from failure until the last set?

How many times a week will you go heavy on a lift assuming you feel alright not piss or great
