There are intersex people though. And it is also possible for something to go wrong in the brain during development and have someone who is mentally female in a phenotypically male body or vice versa. These would likely be "trans".

Until then, therapies like hormones or surgeries may be beneficial for some and not others. It's a touchy subject, but doctors have looked at this and think the good outweighs the bad in offering these trans hormones/surgeries.

But
Should we be cutting off the dinks and removing vaginal canals from people (and children!) who may have a mental illness? Do we do it for people with other forms of body dysmorphia?
Should we be changing language and definitions to avoid making less than 1% of the population uncomfortable? Conflating sex and gender for instance.
Should we be removing women's rights to request female only spaces, care, prisons, and sports? Especially traumatised or religious women who are already vulnerable. Baring in mind statistics show male transgender retain male pattern offending and massive strength advantages over females regardless of HRT?
Should we be skewing census and crime statistics which should allow for accurate data gathering on the population? Male crimes are now being recorded as female here if they self-ID as women.
Should we be denying biological reality and policing speech that pertains to biological reality as hate speech? The different sexes have very different health and medical needs, it is dangerous to blur the lines between them.
Or we could let them dress how they want, call themselves what they want but keep sex-segregation where it is necessary for the safety, dignity and privacy of both sexes. A third space and resources should be created for those who are uncomfortable using the facilities for their birth sex for whatever reason.

Sports is another messed up area. To be blunt sports are stupid. It was always about genetics. The issue is when we make 2 distinct camps, it doesn't properly dichotomize the species. There will always be fringe females with high T or XY female phenotypes that would dominate female sports. It is pretty bullchit that a dude can fake being a woman and win at women's sports. I would create a tiered system based on hormone levels and weight. Such a system would be more competitive and somewhat more fair. Edge cases would still exist though. We do weight classes in UFC and it works similarly. But maybe we should reevaluate why we care so much about sport performance if someone can just take hormones and win...
You could make a tranny freakshow league, but understandably, everyone would fraud and avoid playing in that league. There is no nice answer to this while keeping things the way they are. There was that olympic female runner who turned out to be XY...she had androgen insensitivity...she identified and behaved and looked exactly like a female. How do you deal with people like that? Certainly the y chromosome helped her somewhat. But is she a female or not? It's a grey area. And the grey areas are winning the competitions due to their advantages over pure females.
One thing is for sure. We aren't going back to the old days of putting these people to death or pretending they don't exist at all. That is not happening.

Trans people are mentally ill and should be treated as such. They deserve compassion and understanding and pandering to their mental illness does not help them or society.

A 20lb man and a 200lb women would look very different regardless of hormone levels. Aside from the fat storage and muscle density, men have bigger, thicker tendons, higher bone density, larger lung capacity, stronger hand grip, different pelvic bone movement, more fast twitch muscle fibres.... and a multitude of other advantages. Women would be all but removed from competition. Check out high school boys running times vs olympic females.... it’s very eye-opening. I would either allow a third group as either “open” or “mixed”, it likely would be dominated by men though, but women would have fair competition in their own sports category. Anyone with a Y chromosome would be barred from the strictly women's category.
What of female prisons, did you research the staggering percentage of transwomen imprisoned for sex related and violent crimes as compared to females! Should female prisons who are mostly there for non-violent crimes have to shower and share a cell with a rapist or sexual predator who will be stronger and more intimidating than them?
I see no disadvantage to well-funded third spaces and facilities, such as sports groups, prisons, health services, rape crisis centres, toilets, changing rooms.... aside from the trans people not wanting them because they wouldn’t feel validated enough. But the majority’s safety, privacy and dignity should come before a tiny group's feelings of validation. Do you think that is a reasonable compromise?

As far as experimenting on children, as I said, it should only be done for those kids for whom it would be a dramatic quality of life improvement. What must be done is that which minimizes overall suicide risk and harm. There needs to be an optimal balance where the likelihood of regret for a surgery is balanced by the benefit. This is impossible to perfectly quantify, but like I said, a thorough screening process could help with this. Also, in the future, people will be able to use biomarkers to really tell if there is a woman's brain trapped in a man's body, etc. Right now we aren't there yet so we need to be very cautious. IDK how things are done in the UK, but I know the US trans policy is not as simple as going and getting snipped - especially not as a minor.
