Every 8-10 weeks or so I take a week off - feels like my body needs time to heal back to 100% before going at it again.

No set time, but usually around 12~ weeks on average. Just take a full week off, no deload training.
Currently at 20 weeks no break though.

Only when I take a week off other responsibilities (I look at staying in shape as a responsibility I have to myself) as well. When I go on vacation, I don’t lift. During the holidays, I don’t lift. Other than that I’m on a regular routine all year.

Totally depends on the intensity. If training in the 50-75% range every four weeks, 80% and above gotta be every three.

Lately I've been driving out of state for gym, so only going twice per week. Haven't taken any regular time off since May. Now that my lifts are getting much heavier again, I'm going to need a week off every 6 or so. Legit feel like I'm going to die after a certain amount of time.
