Rewatch the movie, the "crazy" lady was right about all her predictions. The reason he was saved was because he finally sacrified the whore and the kid.

I actually said out loud right before the ending reveal "Wouldn't it be fukked up if <the ending>?" and then it happened and I was just like

Hated that film immensely first time I watched the ending.
After a while I accepted it though.
