All I saw was that you have an anal fissure.

Anyone who has spent a lifetime of doing stuff is going to show some wear and tear on their body.

Not to the extent of your anal tearing, but at least some normal and expected damage.

Mostly begnign and common findings, perhaps some credence to L4L5 disc bulge if clinical symptoms coincide commensurately.
Otherwise, ignore MRI. MRI is given way too much weight for its findings w/ respect to most pain conditions

This guy knows what he's talking about. I've seen MRIs way worse than yours where the person has none or minimal pain. If your symptoms are too much for you to handle get surgery but if you can work through it do core strengthening and PT

Pain is manageable most days. There are days and nights where walking around is difficult and I don’t sleep much because tossing and turning hurts me enough to wake up.

But they’re infrequent

Avoid surgery at all costs. That’s a can of worms you don’t want to open unless 100% absolutely necessary.

Get multiple opinions and seek help also from corrective and strength coaches.

Mine have rarely reached these levels

There have been a handful of times where I’ve needed to be helped out of bed. But again it’s infrequent

I literally have no l4 and l5 discs. Had a bad blowout from heavy lifting and bad form. That was 7 years ago. Get flare ups 3-4 times a year where I'm incapacitated for 3 days and then have about 2 weeks before it gets 95-100% better. A bulging disc is a cake walk. Don't be dramatic, but also be smart about form.

There are days where I sleep in 15 minute increments for a total of 2 hours because of the crippling pain. I've actually been in tears with 11/10 pain getting taken in for an injection that doesn't help. Thankfully the extremely bad flare ups aren't more than once a year.
