It depends, I usually pay. However, I'm usually going somewhere like a biergarten, so it's paying for 2 $6 beers.

Depends. If I don't want to see the girl again, I split. If I want to see her again and know she does too, I'll tell her she will get the next one and pick up the bill.
Never had a problem with her reneging on the second date, in fact it usually results in an invite to her place to have dinner cooked and instasmash.
