Bad diet, bad training + alcohol probs involved.
I went through a heavy drinking period that almost lasted 2 years where I would just maintain my weight (was around 15% bf) every week and was always bloated from drinking. Plus I think a lot more people than you probably think don't know a lot about the science of bodybuilding like calories in vs calories out, bulking/cutting etc.

Too many beers is probably a big part of it. Most people don't realize how many calories are in it then they eat their normal amount of food which is usually way too much.

Some people have goals, these goals include 6 pack abs, but most "wish" and think they can't have it, so they give up on that and just workout without the right diet. Abs are made in the kitchen.
Some other really don't have 6 pack abs (visibility) as goals, they just train to be strong like power-lifters.
It all depends on everyone's goals.
Remember, strongest people on Earth don't have visible 6 pack, they just have a sheer amount of strength and an undercover abs.

Because not everyone that goes to the gym is serious about bodybuilding.
At my gym, about 60-70% just show up and go through the motions.
Keeping a flat stomach after 30 is difficult and there are very few people out there that have the commitment to get/maintain one.
