It was attached to the CSM but airlocked(sealed off) until they were to decend to the moon.

At that time 2 astronauts would get in it. The third would stay on the CSM in Lunar orbit.

Then they detach and land. Then they take off less than 24 hrs later. When they are out of it on the moon it's not sustaining life.
