Look at Oregon--99.9% untouched lol

Even during the Cold War the Russkis knew it wasn't even worth wasting a nuke on Oregoncels

I used to play this WW3 custom map on Warcraft III Battle.net
An ICBM in the game from Russia to the Americas took quite a while lol
Invading the Americas in general was really difficult.
Central America created a Bottleneck because you could only put a few troops there.
Canada was the best entry point to invade because it was big space with nothing there. Just build your bases and army.
Asia was another difficult one to invade.
If you the USA, it was easy to invade other countries in the game because you could just build this big Navy and Air Force. It made the Pacific, Europe and South Asia easy to take over.
South America took forever to reach and so was always the last one to conquer in the game.

There is at least one Amazon DOD cloud data center a few hrs from Portland. We would get toasted for that and Intel.

The only reason they'd hit civilian targets is if they believed missle defenses could/would repel attacks on militay targets but not civilian targets. They would also attack infrastructure like major railroad junctions, freeway junctions, Ports, bridges, power plants etc. The Bay Area would be targeted not because of population but because of the infrastructure, including refineries, railroads, bridges and ports.

Deep Woo CT would suggest Russia would use an underwater nuke off the east coast first. Take that FWIW.

Comfortably in the middle. If they nuked Chikongo, that would take care of a lot of problems

If we are talking tactical and Russia is the initial aggressor, they hit a largely unpopulated strategic target or assaulting front(say M1 Abram tank battalion in E Ukraine). US or allies answer with a tach nuke to infrastructure in a Russian controlled city or border area away from what Russia considers "home." It would have to be void of civilians.
Then the war is either over and negotiations start or it goes full nuke.
If full nuke after tactical exchange or at the onset, it's over for basically everyone who lives in the civilized world.
