To look at US numbers you would have to break it out by race and compare past and current numbers.

You already pay for the sickest least healthy people- you either pay via taxation for government provided care OR you pay via higher health insurance premiums and higher charges from providers due to increased demand for services.

The only way to avoid paying for the visibly unhealthy which miscers enjoy banging on about would be to straight up kill them which seems very unfair given that there are plenty of other self-inflicted expensive medical issues which just aren't so obvious by looking at a person [smoking, alcoholism, drug addiction, STDs, metabolic disease not accompanied by obvious obesity, etc.].
