I'd say 50% of the kids 50% of the time.
ETA:
I need to expand on this.
So when they do not understand (some of these kids' levels are low but they do not necessarily have learning disabilities. However, most do have a learning disability), you need to break it down to very simple terms, differentiate, work one-on-one with them, give them frequent breaks from school work, etc.

I know that kid wasn't actually fighting for his life and you know he wasn't, but his brain told him that he was. And his "handlers" as you called them, were trying to keep him safe and to keep themselves safe. That's going to look like he's winning every time, when in reality, as long as he wasn't hurting himself, they were winning. Now if they were fighting with the same motivations and goals as he was, it would have looked very different obviously.
