Taking a med which lowers blood pressure a lot. Keep falling over and it's exhausting to even raise my hands above my head to wash hair, etc.

Do you faint a lot? Mine used to be 90/50 and I fainted often. Now it’s 100/70.

Yikes, doesn't below 80 systolic BP mean you don't have enough blood pressure to pump the blood all the way around your body?
Your body would be prioritising blood flow to the internal organs and brain, good night sweet fingernails.

Are you extremely athletic and in the habit of doing a lot of cardio?

If your doc isnt aware of how low it is, he/she needs to know. Also you should get to a hospital if it doesnt get better soon. Srs.
Edit: I just noticed how high your HR is. Go to the ER brah. Those are shock vitals.

Edit: Sorry i didn't answer your question. I've been on the med for 2 years. The very low BP happened when i first started the med and i was falling all over the place. But after a while iBP returned to normal(ish). I increased the dose recently as I felt that the dose I was on wasn't having the best therapeutic effect. However clearly I can't stay on this dose with a BP like that so am lowering it but it will take a while. Am seriously considering going to the ER given what you said. Oddly my resting heart rate is in the 40s (because of running every day although that's out now because of tibial fracture). I had been walking around quite a bit when I took that reading. Heart rate now is 70.

I have a great life, job and wife. But have severe endogenous depression - I'm not unhappy about anything but clearly I have some kind of neurochemical issue. Have been through every SSRI, SNRI, etc that there is and a whole range of other meds, plus ECT, rTMS and ******** and Nardil is the only med that makes me well. So it kind of is worth dying for because severe clinical deprssion is hell on earth.

How did I know that?!
Because I used to bike huge distances and developed "Athlete's Heart" and it was not pleasant.
The trick, if that is your issue, is to decondition your heart by not running or anything like that. Eventually, your heart rate will go back to normal.
My pulse used to be in the 40s and I had to give cardio up and then just lift. After about a year it was in the 80s and that's too high in my opinion so I started back with cardio and now it's in the low 60s.

OPs BP is deadly low and his HR is high because his organs arent getting enough blood, so his brain tells his heart to beat faster to pump more blood. Basically the opposite of what happens to conditioned athletes.

I love exercise and I am very dedicated to a routine and that can be kind of like an addiction. It's not if it's benefiting you but if you stand up and want to pass out, that's a sign you are doing too much.
The same with your legs, do not wear your body out doing positive stuff.
