Very rare is to to have a pharmaceutical compound that's 'all good'.
In my experience, the stronger the effects of a chemical compound the more serious the potential side effects. I'd presume this is because the body doesn't physiologically discriminate against wanted and unwanted effects.

I have a client who is affected by the rare form of blindness that Ozempic causes. She's been on it for years and will lose sight in one of her eyes. No way to stop it.

I take Wegovy due to my eating disorder and had to go back to a lower dose when I tried the maximum dose due to the nausea. Other than that is has worked pretty well.
