Yeah, that's why some small time community clinics got busted for administering the vaccine without permission from the govt, since it would make it too hard for them to keep track of records.
Providers usually have 24hrs to report to the registry, so it's done in bulk at the end of the work day.

Thing is that it all varies by provider and location so who even knows whats going on now tbh. We have a chit ton of illegals in Texas and yet somehow they're all still getting vaccines without having to show any ID or insurance, so there's obviously no way their info is being reported the the State.

If you’re going to be honest. Tell them legitimate reasons why you do not feel comfortable getting the shot.
Document everything. Do not record them just take notes on the conversation. Dates. Times. Names. Etc.
Sometimes honestly is the best course.
